At Carolina Collective Dance, we are truly honored that your family chose to be part of our studio, and we are sincerely sad to see you go. It means so much that you trusted us with your dancer, and we are grateful for the time you spent with us. We are always striving to grow and improve, and your feedback is incredibly valuable in helping us become the very best we can be. Please feel welcome to share any thoughts or suggestions with us. Your dancer’s time with us has been meaningful, and we are truly grateful for the opportunity to have trained, encouraged, and supported them. We wish your family nothing but success and happiness in whatever comes next, and our doors are always open should you decide to return.

A completed Withdrawal Form is required to initiate our 30-day cancellation notice. Tuition is processed on the 25th of each month for the upcoming month. Depending on the date this form is submitted, additional tuition charges may apply in accordance with the 30-day notice period.
